How best to organize an agency's chart of accounts

I am curious how other agency's organize their chart of accounts.

Whether first by revenue source and then by project type:

41000 Account Management (AM) 41100 AM - Print projects 41200 AM - Interactive

OR

First by project type and then by revenue source:

41000 Print Projects 41100 Account Management-Print 42000 Interactive Projects 42100 Account Management-Interactive

OR

Am I putting way too much thought into this and another way is best.

I am a 10 veteran of C&P and I am experiencing a problem when I print our audit trail. The names of all the G/L accounts have some how converted to have all the same name "Year to Date Profit/Loss" (different G/L#'s and different subtotals). I've printed a financial statement and have ran a chart of accounts lookup and have looked at the setup and the names appear correctly. It only seems to happen when I print the audit trail... Has anybody ever experience this issue or know any way of fixing it? I've tried to refresh the chart of accounts and did a database utility repair and nothing seems to be able to fix it... Any suggestions? Thanks!!!
